Dan’s present performance level
Dan is a fourth-grade student who rhymes well
with his friends and has a good sense of humor. He is good, in general, social
and education science studies and can successfully participate in math
curriculum with no any special education support. He however, faces writing and
reading challenges as well as support in following complicated instructions. He
would, therefore, benefit from intensive written language and reading services
and instructions from an occupational therapist. The therapists will help his
independent reading habits to improve his labored and slow reading. The
specialist will also assist Dan in writing and following complex written and
oral directions. The parents also offer support Dan’s reading at home. Parents
and teachers will have to encourage Dan to read so that he strives for about 30
minutes of independent reading. Parents should also be models of reading
habits. Encourage Dan to summarize orally what he has read and provide
instructing and sufficient materials to capture his interests.
Assessment of instructional and informational
needs
Dan accurately comprehends and reads 95 words
per minute based on the graded passages in the 2nd grade level of his
independent reading level. It means that his reading fluency at 4th grade is
poor. He has an average of 555 words correctly per minute less than the 75%
comprehension. The goal is to enable Dan meet the expected fourth grade reading
level of 90-100% comprehension at a rate of 120-150 words each minute.
The other challenge is with his writing. He
needs help following complex instructions. His written work comprises of lack
of vital punctuations, many spelling errors, and very short sentences of 3 to 5
words. He cannot write a paragraph without help.
Measurable Annual Goals
Annual goal is to assist Dan
improve his reading fluency to a fourth-grade level by May, 2016. There is also
the goal of improving Dan’s reading comprehension to the 4.5-grade level by
May, 2016. He will progress towards these goals through classroom reading
participation, home reading as part of special projects and norm-referenced
test in state reading assessment. Parents will also be motivating and observing
Dan’s reading. Fluency evaluation will also help identify areas for correction

Objective 1: When given a story for 4th-grade
level, Dan will read the story in 2 minutes with an average of 60 to more
words.
Objective 2: When given a
fourth-grade story, Dan will read the story in 2 minutes with 70 words or more
being correct
Objective 3: When assigned a 4th-grade story
Dan will read it in 2 minutes of 75 words or more being correct.

Goal 1: Dan will understand writing strategies
such as editing, revising and planning compositions
Goal 2: Summarization: by
May 2016, Dan should understand how to summarize texts
Goal 3: By May 2016. Dan
will take part in collaborative writing involving participation with a peer in
editing, drafting and revising compositions.
Goal 4: Dan will understand word processes
through the use of computer alongside instructional support in writing of
assignments.
Goal 5: Dan will understand

Least restrictive
environment and service placement goal
Dan will receive over 60% of his writing
support in the classroom and another more than 60% in reading at home. The
environment can be modified to support his below-level writing and reading
skills. Thus, services can be provided in a residential facility, special education
day schools, general education class or special classes. Classroom modification
includes ensuring that the right books and assignments are available. There is
also the need to provide additional time during in-class tests and quizzes.
Assignments should involve family for early preparations and homework
activities.
